Effective July 1, 2011
* An administrative fee of $5 per patient
will be charged for immunization clinic visits. This fee is in addition to
standard immunization costs. Please contact the JACOHD for pricing
information.
** An
administrative fee of $10 per patient will be charged for STD clinic visits, with a nurse
practitioner.
